Medical Exam: “Fit” 16/07/18 at London, ON. Age: 29 yrs. Attestation: 16/07/1916 at London, ON. He was reported ill in England November, 1918.

6/11/18 Influenza at Halifax NS. 6/11/18 – 31/12/18 “Form of Will”. “My WILL is in a file at the Canadian Bank of Commerce – Chatham, On. Certificate of Service: Cdn. Chaplain Service 16/07/18. In Canada with Cdn. Chaplain Service 28/02/19 – 13/03/ 1920 

Medical History: at London, ON. Born at Guelph, Wellington Co.. Height: 5’ 6” , Girth: 54”, Weight- 144. Vaccinated, Vision 20/20 and Hearing – Good.     

He came down with influenza and Pneumonia 7/10/18:-

“ He was critically ill with Pneumonia following Influenza an his life was despaired of. He is still in highly strung nervous condition – any sudden noise or excitement startles him and accelerates, the pulse rates. He is troubled with sleeplessness. The pulse is lacking in tones, and easily compressible. The heart sounds are normal. He perspires and flushed on slight excitement and he complains of lassitude and lack of vigour. The lungs show nothing abnormal. The tongue is furred, and ha slight nervous dyspepsia.”   

He had six weeks at Camp Hill Hospital – Halifax NS. and 4 weeks at Pine Hill – Halifax – 2 weeks. 21/11/18 at Halifax NS. (26/11/19).
