Apparent Age: 23 yrs. 9 mths; Height 5’ 8”, Girth: 34” Complexion: Fresh, Eyes: Blue Grey, Hair: Light Brown. “A number of small brown moles scattered around the chest, two small scars over the right knee.” Medical : 8/05/16 at Chatham, ON. Sailed from Halifax, NS. aboard the S. S. Lapland 31 May, 1916 arrived in England, 7/04/17 

28/03/17 in action: No.1 Cdn. Fld. Amb. 7/11/17 , received a GSW to his Left Thigh  and his left Eye. Resulted loss of vision to Left Eye. GSW: left Thigh and left Eye (actually it was a shell injury.  Joined with the 18th Bn. Cdn., Inf. Serving in France 27/09/17 and contracted a partial loss on vision in Left Eye. 15/11/17: Admitted to, No. 10 Fld. Ambulance. Injured Left Thigh and left eye was sent to Chelsea.  

13/03/18 was admitted to No. 5 General Hos. Kirkdale and 15/04/18 invalided to Canada.  Reported a Cataract in Left Eye:- LMCH at London, ON. 17/05/18 

“Discharged from HMS – Medically unfit due to wounds , although firtt for employment in ‘Civil Life’. He was aboard the Hospital Ship”Araguaya”departed for Canada 15/04/18 arrived Canada 25/04/18.

Charles’ discharge Certificate : Served in France with the 18th Bn. “Category “E” – medically Unfit due to wounds although fit for employment. Age: 25 yrs 

“The marginally named man gave a history of having had a left eye wounded 16/11/17 at Ypres. He has a traumatic cataract in his left eye. This has twice needled here and will require further operative procedure.  

Proceeding to Discharge – Discharge 17/06/18 at London ON.   His statement  – was two months in France. Was gassed the night of 5/11/17 but carried on – wounded 6/21/17 GSW to left Eye and left Thigh – West Cliffe Hospital 26/11/17 – Traumatic Cataract. GSW Left Eye operated on Dec. last 1917 and 3/01/18
