Reported Wounded 18/08/17 – ‘gas poisoning 18/08/17 – Suffering from Gas poisoning – noted to Mrs. Fanny Joyce (RNS). 21/O8/18 – Adm: 12 Gen. Hosp. – ROUEN 16/08/1918 – Arm &  Chest – War. Hosp. Rubery Hill – 31/08/18 to S.W. Right Arm & Chest – MD1 – CANADA 18/08/17 to 25/08/17.

12/08/18 – GSW – Chest – through and through wound passing through Rt. Lung and passing out over inner body – RT. Scapula. He had Luimoptysis for GWH – Wounds healed in two months. There is a small circular scar over 5th Rib in ant. Auxiliary – line of right chest healed, and a circular scar size of a ‘silver dollar over the upper part of inner border of Rt. Scapula – healed. There is a healed scar 1 ½” x 1” inner surface upper 1/3 right arm and Lung. Gas 18/08/17 

12/08/18 – Shrapnel and Gas – “No wound be unable to blow glass, on account of lung condition – Discharge as Medically Unfit.  22/08/18 “ – Invalided – Wounded and posted to Quebec – Regt. – Bramshott – W3083 – Part 11 – D. O. 90 3/08/18. Served in Canada, England and France (with 5th Cdn. – Mounted).  

He states he has lost 20 lbs. since being wounded in the thumb and index fingers – Grip – 75 % of normal. Joint movements – normal “Weight: 133# – at London, Ontario – 29/05/19 – Pulm. Debility.
